Track My Physical Activity

Keep track of all of the exercise you do each day with this scorecard. You can record any type of exercise you did such as playing soccer, basketball, jumping rope, playing dodge ball, hop-scotch, as well any other sport or game by using the little star that represents 10 minutes. Keep track of the activities you do each day and you’ll be able to track your progress each week and month! Shoot for the stars and try to have between 3-6 stars on your scorecard each day! When you get enough exercise everyday, your body stays healthy and your muscles stay fresh!

This scorecard allows you to keep track of all the food you eat everyday. Healthy minds and bodies need to eat healthy foods! Eating meals that have a balance of the different food groups can help keep your body healthy for the fun activities you like to do or help your brain work more efficiently. Your scorecard will tell you how much of each food group you should be eating and help you keep track of the foods you that you eat. You will be able to track how well you do at eating the recommended amount of each food group each week and month! Also remember to make your plate as colorful as possible to keep your body at its healthiest! Get your parents to help you with tracking the foods that you eat!
